Flu Vaccine Clinic Dates

Vaccinations for UAMS employees, volunteers, non-employees and students will be available at special flu vaccine clinics prior to flu season.

Consent Form Required

The deadline for compliance with the influenza vaccination requirement is Nov. 17, 2023. You will need to fill out a consent form before receiving your flu shot.

Please note: This form can only be accessed when connected to the UAMS network (or through VPN) and the form submission is only valid for 48 hours prior to receiving the vaccine.

Location

The location for all flu vaccine clinics will be at the main UAMS campus in Little Rock, on the ground floor corridor of the Education II Building, in the hallway closest to the entrance to Parking Deck 2.
