Flu Vaccination Requirement
Flu vaccinations are mandatory (HR.4.5.18) for all UAMS employees, volunteers, non-employees and students.
According to CDC guidelines, the influenza vaccination is the best strategy to protect the health of our staff and students and the patients and families we serve.
Where to Get Your Flu Shot
Vaccinations are available to all central Arkansas UAMS employees and students in the Student and Employee Health Services Clinic.
In addition to the standard flu vaccine, Student and Employee Health Services offers Flu-Quad, Flu Blok and high-dose vaccinations.
Vaccinations will also be available at special flu clinics prior to flu season.
More information about the flu vaccine and the student and employee flu shot schedule will be sent by UAMS email.
Employees Outside of Pulaski County
Employees and students working outside of Pulaski County, Arkansas, may receive their vaccine at one of the UAMS Regional Campuses. Please contact the employee health nurse at your local UAMS Regional Campus to arrange for your vaccine.
Complete the Online Flu Vaccination Consent Form
Flu vaccination consent is paperless. Before you get in line to receive a vaccine from a Student and Employee Health Services Clinic, please do the following:
- Complete the online flu consent form no more than 48 hours prior to arriving at the flu vaccine clinic. After 48 hours, the consent will be void and a new consent will need to be completed. Failing to complete the consent prior to arriving will delay your vaccination.
- Bring your UAMS ID badge to the flu vaccination clinic.
Display Your Flu Tag
All employees, non-employees, and students must display a tag on their badge showing that they received a flu shot. These tags will be given during the flu vaccine clinics.
If you receive your vaccination outside of a Student and Employee Health flu clinic, please forward your documentation to studentandemployeehealth@uams.edu along with your slot number so we may send your tag to you via interdepartmental mail. Alternatively, you can bring your documentation to the flu vaccine clinic to receive your tag.
Employees and students who do not get the flu shot and are not granted an exemption will be subject to disciplinary measures.
Who Are Non-Employees?
Non-employees are defined as non-UAMS workers, contractors, visiting students, visiting faculty, pharmaceutical representatives, traveling nurses, locum tenens physicians, custodians, and employees of businesses who work on a UAMS campus.
