COVID-19 Update
New Electronic Medical Records System and Patient Portal
Our clinic has transitioned to a new electronic medical records system through Epic. You will have a new patient portal called UAMS MyChart. It will keep all of your provider’s notes and test and lab results from all UAMS sites.
You will still have access to your MyPortal account, but no more information will be added to this account once our clinic begins using UAMS MyChart. You will be able to see all your most recent health information in your UAMS MyChart account.

Before Coming to a UAMS Health Location
If you have any symptoms of COVID-19, think you may have been exposed to someone with COVID-19, or have tested positive for COVID-19, you will need to reschedule your appointment.
Patients who have had a COVID-19 infection may have in-person appointments 21 days after infection or a positive COVID test, as long as your symptoms are improving.
Call the UAMS Health appointment center at (501) 686-8000 to reschedule your appointment or ask if your appointment can be done by phone or video. Please call your clinic if you have questions about coming for an in-person visit.
What to Expect at Your Appointment
Everyone who comes to a UAMS Health location is required to wear a mask and pass a health screening. You will also have your temperature taken. Please do not remove your mask until asked to do so by a UAMS Health employee.
Visitors required or recommended to quarantine by the Arkansas Department of Health (ADH) due to illness, potential exposure, travel or other reason are not allowed at UAMS Health locations until cleared by ADH.
Outpatient Visitor Guidelines
One visitor may come to your appointment with you. You may also include others in your appointment by phone or video chat. Visitors must be 18 or older. Some clinics have additional visitor guidelines. Call your clinic for more information if you have a visitation question.

About Family Medical Center in Fayetteville
The UAMS Family Medical Center, Fayetteville provides primary care medical services to patients of all ages. We not only offer treatment for acute medical conditions but also ongoing treatment for chronic conditions. We cover all stages of life from pediatric conditions to senior health care. We also offer gynecology and obstetric services.
The clinic is able to perform minor office surgical procedures in clinic. We offer many tests in house including, pregnancy, strep, flu, mononucleosis and TB skin. Our clinic also provides quality training experiences in your community to ensure that medical staff members are educated to serve residents of Fayetteville and Northwest Arkansas in the future. Our caring staff serves a six-county region that includes Benton, Boone, Carroll, Madison, Newton and Washington.
Transplant Services
Our transplant services allow patients who live in northwest Arkansas to participate in the UAMS transplant program locally instead of traveling to Little Rock. We treat post-transplant patients in need of follow-up care. Our clinic schedules patients one Friday per month for follow-ups to kidney and liver transplants. Affiliation
Our resident physicians provide care for our patients in one of two local hospitals: Washington Regional Medical Center in Fayetteville and Northwest Medical Center in Springdale.
