COVID-19 Update
New Electronic Medical Records System and Patient Portal
Our clinic has transitioned to a new electronic medical records system through Epic. You will have a new patient portal called UAMS MyChart. It will keep all of your provider’s notes and test and lab results from all UAMS sites.
You will still have access to your MyPortal account, but no more information will be added to this account once our clinic begins using UAMS MyChart. You will be able to see all your most recent health information in your UAMS MyChart account.
- Access UAMS MyChart (new portal)
- Access MyPortal (retiring portal)
- Learn more about electronic medical records at UAMS Health.
Before Coming to a UAMS Health Location
If you have any symptoms of COVID-19, think you may have been exposed to someone with COVID-19, or have tested positive for COVID-19, you will need to reschedule your appointment.
Patients who have had a COVID-19 infection may have in-person appointments 21 days after infection or a positive COVID test, as long as your symptoms are improving.
Call the UAMS Health appointment center at (501) 686-8000 to reschedule your appointment or ask if your appointment can be done by phone or video. Please call your clinic if you have questions about coming for an in-person visit.
What to Expect at Your Appointment
Everyone who comes to a UAMS Health location is required to wear a mask and pass a health screening. You will also have your temperature taken. Please do not remove your mask until asked to do so by a UAMS Health employee.
Visitors required or recommended to quarantine by the Arkansas Department of Health (ADH) due to illness, potential exposure, travel or other reason are not allowed at UAMS Health locations until cleared by ADH.
Outpatient Visitor Guidelines
One visitor may come to your appointment with you. You may also include others in your appointment by phone or video chat. Visitors must be 18 or older. Some clinics have additional visitor guidelines. Call your clinic for more information if you have a visitation question.
Please visit our COVID-19 resources to learn more about what to expect at your appointment.

Prescription Information
If you need a prescription refilled, please call your pharmacy, and allow us 48 hours to process. If we need to see you before we can refill a prescription, we will call you. Appointment Information
Opioid Policy
We do not prescribe pain medicines to anyone on a first visit to our clinic.
If you feel you need a narcotic, we will provide information on our opioid treatment program. If you agree to participate, you will be asked to sign an Opioid Treatment Agreement and we will do all we can to help you. We will ask for a urine sample on your first visit and on future visits, as needed. Our staff will monitor the Arkansas Prescription Monitoring Programs (ArPAP). To be accepted and remain in the Opioid Treatment Program, your doctor will review your past medical records, the ArPMP report, and results of your urine drug screens. You will be required to attend routine office visits and participate in the treatment plan in order to continue in the program.
We do not prescribe high risk medicines, such as Benzodiazepines, muscle relaxers or hypnotics when we prescribe opioids.
If you have any questions, please call our clinic at 870-572-6507. We look forward to taking care of you, your family, and all your healthcare needs.
What to Bring to Your Appointment
Please fill out and bring all new patient forms with you to the clinic for your first visit. We will also need a copy of your driver’s license (or another photo ID) and a copy of your insurance cards.
For your first appointment, please check in 15 minutes before your appointment time. We will need to make sure we have your correct contact information, date of birth, address, a working phone number, and email, if you have one. We will need to verify your insurance. All co-payments and deductibles are due when you check in for your appointment.
Your first visit will be your longest visit, as we will need to enter your information into our Electronic Medical Record. This visit will include a physical exam and we will go over your medical history. Please bring with you all medicines that you take (in their original bottles), including prescriptions, over the counter, vitamins, and herbal supplements.
Telemedicine Information
In an effort to keep our patients and health care workers safe, we are offering virtual visits and phone visits.
This service is available to both new and existing patients. Patients should call 870-572-6507 to schedule a telemedicine appointment.
